What to Bring to Your First Appointment:

  1. Health Insurance Plan Card
  2. Driver's License (or Other Government Issue Photo ID)
  3. Written List of all your Current Medications
  4. Written List of all your Medication Allergies
  5. List of any recent Imaging of your Head or Neck (including CT Scans, MRI Scans, Ultrasounds, etc)
  6. Completed New Patient Registration Form. You may also fill this form out when you arrive if you prefer.
